BronxCare is the largest voluntary, not-for-profit health and teaching hospital system serving the South and Central Bronx, with 859 beds and more than 4,500 employees. BronxCare is now among the largest providers of outpatient services in New York City, with close to one million visits annually. BronxCare maintains a large residency and fellowship program with an affiliation with the Icahn School of Medicine at Mount Sinai and Clinical collaboration with the Mount Sinai Health System.

• Provides quality diabetes self-management education and medical nutrition therapy in individual and/or group settings for patients in our Ambulatory Women’s Health Center diagnosed with diabetes in pregnancy (gestational/pre-existing Type 1 or Type 2)
• Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes.
• Documents all individual contacts/visits in the Electronic Health Record and outcomes database according to the guidelines in a timely manner.
• Applies current research and evidence-based care to practice.
• Collaborates, advocates, and confers with other members of the interdisciplinary care team in developing person-centered diabetes plans.
• Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.
• Collaborates with community partners to establish and maintain on-going support options.
• Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
• Participates in meetings, journal clubs, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.
• Performs other duties as assigned.

Bachelor’s degree in dietetics/nutrition.

Experience

Diabetes self-management education and support experience preferred.

License/Certification/Registration

• Valid RD/N credential
• Current NYS CDN
• Certified Diabetes Care and Education Specialist or BC-ADM credential.
