Overview

Responsible for developing, cultivating and maintaining relationships with families within the Health Families Program through home visitation.

Responsibilities

- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.

 

 

 

- Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.

 

 

 

- Conducts family assessment to determine the needs of the Family and develop goals.

 

 

 

- Conducts Parent Surveys to determine the needs of the Family and develop goals.

 

 


- Makes visits to home of families according to the level of the family and frequency required as per the HFA standards. Flexibility is required.

 

 

 

- Accompanies families to agencies, organization and medical visits, when necessary.

 

 

 

- Conducts assessment of families and develops with the input of the family goals and outcomes based on services needed.

 

 

 

- Responsible for tracking and generating written reports as they pertain to all service delivery in case records and in other required data collection instrument.

 

 

 

- Assists in special projects and perform other duties as delegated by the Program Manager and Supervisor.

 

 

 


- Attends all required healthy families training required to fulfill the position and to build on skills. Travel may be required in state and out of state.

Qualifications

- At least one (1) year experience working within child care facility such as Day Care Center or Head Start Program.

 

 

- Licensure appropriate to degree – i.e. MSW or CSW 

 

 

- Certificate Program in Child Care

 

 

- Associates in Psychology, Social Work, Elementary Education

 

 

- Computer literate
