Overview

The BronxCare Health System Department of Family Medicine provides a range of health care services for the South Bronx community.  We are a multidisciplinary team of social workers, physicians, nurses, community health workers, and integrative health specialists dedicated to health care that is patient-centered and addresses the needs of the whole person and community.  We are an exceptional academic program, training future family medicine physicians in the biopsychosocial model of care and leading innovative initiatives to address complex healthcare needs and improve access to mental health and substance use treatment.  We are seeking a dedicated clinical social worker to join our dynamic faculty as part of the behavioral health team. 

Responsibilities

  • Provide clinical social work services to a diverse caseload of adults and children in our outpatient primary care and specialty clinics.
  • Services include individual, group, and family psychotherapy, psychosocial assessments, crisis intervention, case management, and referrals.
  • Utilize Collaborative Care and SBIRT models for improving access to mental health and substance use treatment.
  • Utilize evidence-based psychotherapy including cognitive behavioral therapy (CBT), interpersonal psychotherapy, dialectical behavior therapy (DBT) and motivational interviewing.
  • Facilitate and develop medical resident education curriculum and activities regarding behavioral medicine, communication and interpersonal skills and well-being.
  • Work with a multidisciplinary team to provide evidence-based, trauma-informed and culturally competent care.

Qualifications

  • Current NYS social work license (LMSW/LCSW preferred)
  • Fluent in Spanish
  • Excellent engagement, assessment, and documentation skills
  • Excellent communication skills, including written and presentation skills
  • Ability to multitask and prioritize
  • Ability to work in fast-paced healthcare environment
  • Leadership qualities
  • Experience teaching/facilitating workshops.
