Overview

Serve as the Healthy Families services Coordinator for the Maternal Infant and Early Childhood Home Visiting Services and for all children and families enrolled in the program. Coordinates staff in the implementation of the MIECHV benchmarks/state performance targets as required by the grant. Assure that all Healthy Families America/New York best practice standards are met. Will make home visits to ensure fidelity of the home visiting model. Ensure that all activities are in adherence with all state requirements and grant regulations.

Responsibilities

Contribution and participation includes data collection, analysis, implementation of and compliance
with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service, (e) Employee adherence to the Organization’s Policy, Procedures and Regulations.

Responsible for coordinating staff in the implementation of the MIECHV benchmarks/state performance targets as required by the grant.Coordinate staff in developing and implementing all Parent-child interaction activities with the use of the evidence-based curriculum and will be responsible for tracking all data that is required to be collected and entered into the healthy families MIS system Coordinate and ensure that all children (0-5) are monitored in their development. Ensure that staff is conducting developmental screens, and referrals are done within Healthy Families guidelines. Act as a liaison between Early Intervention/CPSE/CSE and the healthy families program. Will facilitate groups for families and children that we serve in order to meet the MIECHV benchmarks and Continuous Quality Improvement requirements.

Coordinate all programming related to Continuous quality improvement/Bench Marks related to the MIECHV/Healthy Families grant. Will assist the program director with the training/ direct supervision of healthy families supervisors and provide coverage when necessary according to the best practice standards. Attend all program related meetings/training and represent the program director when she is not available to attend state /regional meetings. Travel is required.

Qualifications

Prior Social Work or Education experience working with infants and toddlers.
Prior experience in supervising staff preferred.
3 or more years of experience working with children and families in a home visiting program.
Bachelor’s degree in social work/ childhood education or other human services related field with at least 2
years post experience is required.
Master’s degree/ or current Master’s program enrollment preferred in an accredited school for early
childhood education/social work, or other related field such as human services or psychology.
