Overview

The Program Supervisor of Healthy Families Program reports directly to the Program Director. The Program Supervisor is responsible for providing direct supervision to the Family Support and Family Assessment workers.

Responsibilities

Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.Contributes to and participates in the Performance/Quality Improvement activities of the assigned department.

Contribution and participation includes data collection, analysis, implementation of and compliance with
risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and d) Patient and Customer Service.

The Program Supervisor may represent the Program Director in project related meetings.Will work with Program Director to develop specific plans for the implementation of program activates.Will with the Program Director recruit, hire and train the Family Assessment and Family Support Workers.Responsible for tracking and evaluating the activities of staff, including the previewing of staff case records and generating written reports as they pertain to staff functions and program operation.

Assists in the coordination and implementation of staff training with the Program Director.Makes home visits to supervise staff and assess families, when necessary.Responsible for providing direct individual ( 2 hours per Worker per week) and group supervision to Family Support and Assessment workers.Ensures that the goals of Healthy Families program are met. Assists with Special projects and perform other duties as delegated by the Program Director and OB/GYN Administrative staff.

Qualifications

 

An Associates degree with Commensurate Healthy Families home visiting services has been added as being required for this position.

A Bachelor's degree with 3 years of experience working with children and families is  preferred 

A Master's Degree is preferred 

Prior Social work or Public Health experience
Prior experience in a supervising staff and or conducting home visits
Concentration in Health and or Administration with three (3) years’ experience
