Provide data abstraction, entry and reporting utilizing URS (Universal Reporting System). Act as liaison with Pediatric ID case mangers to assure all forms are completed on a timely basis. Ensure all data requirements of NY State AIDS Institute and Ryan White grants are met.
- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. - Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the Bronx-Lebanon Hospital Center (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.
- Manages data/information by performing accurate data abstraction and entry and electronic submission of required reports (URS) while maintaining confidentiality.
- Acts as liaison between Pediatric ID and social workers to assure all case management data, including initial Assessments, follow-ups, etc., are entered into URS in a timely manner.
- Acts as liaison between Pediatric ID and nutritionist to assure all data, including Initial Assessments, follow-ups, etc., are entered into URS in a timely manner.
- Maintains accuracy in all patient related information, updates patients’ demographic/contact information for charting and data collection purposes and maintains confidentiality.
- Coordinates filing of all necessary data into the medical record to insure compliance with AIDS Institute requirements.
- Collects and analyzes statistical information, keeps accurate records and provides tables and documents for reports and presentations.
- Provides support to office operations including filing, copying, faxing, word processing and other related duties.
- Collaborates with members of Department of OB/GYN who are engaged in similar data and tracking activities.
- Performs other duties and participates in other projects as assigned by the Chief of Pediatric ID, the Administrator of Pediatric ID, or other appropriate staff.
- Good organization skills; must be detailed oriented
- Very accurate typing
- Knowledge of Unified Reporting System (URS) a plus
- Bilingual (Spanish preferred)
- High School or GED
- Associate’s
- Computer expertise in DOS and Windows, Microsoft Office including MS Access
