“The BronxCare Health System Department of Population Health seeks an Outreach Team Assistant Coordinator to help us improve the quality of care for our patients. The Outreach Team is responsible for contacting patients by phone and mail to assist with scheduling needed appointments (e.g., primary care follow-up, cancer screening tests such as mammograms, diabetes eye exam visits, etc.). The Outreach Team Assistant Coordinator interacts with patients professionally, courteously, and in a culturally-sensitive manner. The Outreach Team uses the electronic medical record to determine which patients should be contacted and to document their outreach efforts. Training and support in these activities will be provided. Excellent verbal and listening skills for communicating clearly and effectively are required. Experience in a health care setting working with patients and bilingual (English/Spanish) fluency is preferred.”
The EMR Educator will be responsible of the BronxCare Health System/MLK Jr. Electronic Health Record
Clinician Training program for creating training materials and for educating outpatient and inpatient department
staff. To perform job-specific tasks in the Allscripts EHR and toward improving EHR documentation, improving
patient safety, reducing bill holds, denials and ensuring compliance with regulatory requirements
Performs various tasks involved in receiving, storing and distributing materials,
equipment and products within the Hospital Center.
Performs and obtains high quality radiographs on all patients (i.e. In-Patients, Emergency Room Patients or Private Out Patients); recognizes the need for and utilizes lead shielding where appropriate in accordance with departmental policies. Develops all X-rays when required. Provides patient comfort (e.g., bedpans, blankets) as required. Performs required clerical work when necessary to maintain patient records. Maintains a clean and orderly environment to include Radiology Room, Radiology Equipment, and Radiology Accessories. Informs appropriate personnel when equipment is in need of repair.
